Intense Pulsed Light (IPL) therapy is what we offer to rejuvenate sun damaged skin, remove pigment, diminish redness,and reduce rosacea. There is an addition to the procedure that can substantially enhance the results of the IPL photofacial known as Photodynamic Therapy, or PDT. For this treatment we add a photosensitizing agent in the form of topical cream that is applied to the area undergoing the photofacial. The photosensitizing agent we use is Levulan (aminolevulinic acid). Levulan is absorbed selectively by those cells in the skin that produce pigment; the blood vessels that make our skin red and cause rosacea; superficial premalignant skin lesions known as actinic Keratosis (AK’s); and cells infected with acne causing bacteria. Levulan multiplies the effect and thereby the benefits of the photo facial treatment by targeting these specific cells for destruction. The result is significant improvement in actinically damaged skin, sun damaged skin, and acne.

Preparation before treatment

You must not tan the treatment areas before treatment and avoid self tanners for 2 weeks prior to your appointment. If you are susceptible to cold sores you should take your antiviral medication to prevent an outbreak.

Description of Photodynamic Therapy Procedure:

  • A medical professional will apply the medicine.
  • After 1-2 hours Levulan will be activated with IPL light source.
  • You will be provided protective eyewear during the treatment.
  • It is normal to feel a stinging, tingling, prickling, or burning of the lesions, but this should go away after the treatment.
  • Treatments take about 45 minutes.
  • There is some discomfort by no significant pain.

What should I expect after treatment?

The pigmented spots and capillaries treated may appear as darker flecks or temporarily crust over. You will also notice a “flushed” look that soon fades as capillaries begin to vanish and pigmented spots slough off, leaving the skin looking younger and smother. The treated area should be kept clean, however, be careful not to aggressively rub or scrub the area or use harsh cleansers for the first few days after your treatment. After 1 week, there might be some residual redness that will soon subside and can be covered with makeup.

What is the down time with IPL photo facial and Levulan?

You can expect 3-7 days of redness and mild to moderate swelling of the treated area followed by light peeling. The intensity of the post treatment redness and swelling depends on how long the Levulan is left on prior to the IPL photo facial; the individual’s sensitivity to Levulan; the degree of sun damage; active acne; and other underlying skin conditions that may be present. As with the standard IPL photo facial, pigmented areas of the treated skin will darken temporarily and disappear in a few days after the treatment.

Although you do not look your best immediately after the treatment, your activities are not limited as long as you either avoid direct sunlight entirely or protect yourself from the sun for 48 hours following treatment. We recommend the use of a good sunscreen, preferably 30-60 SPF. Protection from sun is important because the Levulan treatment will sensitize your skin to UV rays and can cause severe sunburn.

How soon will I see results of the treatment?

The results will begin to be apparent as the post treatment redness and swelling subside in 5-7 days. After this time, active acne resolves, rosacea improves, pigmentation lightens, and chronic redness reduces. Within two weeks after a PDT treatment, the skin feel noticeably smoother. Any actinic keratosis will have largely disappeared. Within months of a series of the IPL photo facial with Leulan treatments, fine lines and wrinkles show definite improvement.

How many treatments are required?

Although there is increased post treatment downtime with the Levulan, as compared with other standard treatments, fewer treatments are required to achieve remarkable results. For active acne, facial rejuvenation from sun damaged, heavy pigmentation , and rosacea, three treatments are usually all that are necessary. Some patients are completely satisfied after only one treatment.
